1. No purchase necessary
To enter, you must complete the NurseVillage entry form. One entry per person
per month. This offer is valid only through the NurseVillage entry form located
on our Internet site.
2. Entry deadline
To be eligible for the monthly prize, all entries must be received no later than
midnight on day of each month. Drawings will be held on the first business
day of each month. NurseVillage is not responsible for lost, late, misdirected,
or incomplete email. All information must be completely filled out in order
to be eligible for the prizes.
3. Notification
Winners will be notified by email at the email address shown on the entry form.
4. Prizes
One contest prize winner will receive the monthly designated prize. Certain restrictions
apply. The value of these prizes will not exceed $500. All prizes are non-transferable
and no substitutions will be made. Winners must provide their social security
number and will be responsible for the payment of all income taxes and any
other state, local, or federal taxes payable in connection with the prize received.
Acceptance of prize offered constitutes permission to use winners name and/or
photo for purposes of advertising and promotion without further compensation.
By accepting a prize, winners agree to release and hold NurseVillage and all
of its affiliates harmless from any and all losses, damages, rights, claims
and actions of any kind in conjunction with the drawing or any prizes. These
prizes cannot be used in conjunction with any other promotion or offer. Expenses
not specifically stated are the sole responsibility of the winner. Chances
of winning a prize are determined by the number of eligible entries received.
5. Acceptance of Rules and Regulations
By submitting an entry, each entrant agrees to and is deemed to have accepted
all of the terms, conditions, rules and regulations contained herein.
